Common use
Ciloxan ophthalmic solution is indicated for the treatment of infections caused by susceptible strains of the designated microorganisms in the conditions listed below:
- Bacterial conjunctivitis: Treatment of conjunctival inflammation and purulent discharge caused by susceptible strains of Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus pneumoniae, Enterobacter cloacae, Haemophilus influenzae, Proteus mirabilis, Pseudomonas aeruginosa, and Serratia marcescens
- Corneal ulcers: Management of bacterial keratitis involving the corneal epithelium and stroma, particularly effective against Pseudomonas species and other gram-negative organisms
- Off-label uses may include perioperative prophylaxis in ocular surgery and treatment of blepharitis when bacterial involvement is confirmed
Dosage and direction
For bacterial conjunctivitis:
- Instill 1-2 drops into the affected eye(s) every 2 hours while awake for the first 2 days
- Thereafter, instill 1-2 drops every 4 hours while awake for the next 5 days
- Total treatment duration should not exceed 7 days
For corneal ulcers:
- Instill 2 drops into the affected eye every 15 minutes for the first 6 hours
- Then instill 2 drops every 30 minutes for the remainder of the first day
- On day 2, instill 2 drops hourly
- From day 3 through day 14, instill 2 drops every 4 hours
- Treatment may be extended beyond 14 days if corneal re-epithelialization has not occurred
Administration technique:
- Wash hands thoroughly before administration
- Tilt head backward and pull lower eyelid downward to form a pouch
- Instill prescribed number of drops into the conjunctival sac
- Avoid touching dropper tip to any surface to prevent contamination
- Close eyes gently for 1-2 minutes and apply gentle pressure to lacrimal sac
- Wait at least 5 minutes before instilling any other ophthalmic medications
Precautions
- Not for injection or oral use; for topical ophthalmic use only
- Contact lenses should not be worn during treatment for bacterial conjunctivitis
- Patients should be advised not to wear contact lenses if they have signs and symptoms of bacterial conjunctivitis
- Benzalkonium chloride preservative may be absorbed by soft contact lenses and cause discoloration
- May cause temporary blurring of vision immediately after installation; patients should not drive or operate machinery until vision clears
- Superinfections may occur with prolonged use due to nonsusceptible organisms including fungi
- Use with caution in patients with history of hypersensitivity to other quinolones
- Pregnancy Category C: Should be used during pregnancy only if potential benefit justifies potential risk to fetus
Contraindications
- History of hypersensitivity to ciprofloxacin, other quinolones, or any component of the formulation
- Viral keratitis including herpes simplex keratitis
- Mycobacterial or fungal infections of the eye
- Use in children under 1 year of age (safety and effectiveness not established)
- Patients with known hypersensitivity to benzalkonium chloride
Possible side effects
Ocular adverse reactions (≥1%):
- Transient ocular burning/stinging upon installation (5-10%)
- Conjunctival hyperemia (3-5%)
- Crystalline precipitates (1-3%)
- Foreign body sensation (1-2%)
- Itching, photophobia, corneal staining, lacrimation, eyelid edema (each <1%)
Less common but serious reactions:
- Allergic reactions including facial edema, urticaria, itching
- White crystalline precipitates in superficial portion of corneal defect (usually resolve without sequelae)
- Corneal infiltrates, punctate keratitis
- Nausea, dizziness, and headache (systemic absorption)
- Taste disturbance following nasolacrimal drainage
Drug interaction
- Specific ophthalmic drug interaction studies have not been conducted
- Theoretical interactions with metal cation-containing preparations (zinc, iron, calcium) may reduce absorption
- Separation of administration by at least 5 minutes recommended when using multiple ophthalmic products
- May potentiate effects of oral anticoagulants when systemic absorption occurs
- Concomitant use with corticosteroids may mask infection or enhance microbial proliferation
Missed dose
- If a dose is missed, administer as soon as remembered
- If it is almost time for the next dose, skip the missed dose and resume regular dosing schedule
- Do not double the dose to make up for missed administration
- Maintain as consistent a dosing schedule as possible throughout treatment period
Overdose
- Topical ocular overdose is unlikely due to limited systemic absorption
- Excessive administration may be flushed from eye(s) with warm tap water
- Systemic effects due to accidental ingestion may include nausea, vomiting, diarrhea, headache, dizziness
- In case of accidental ingestion, seek medical attention or contact poison control center
- No specific antidote exists; treatment should be symptomatic and supportive
Storage
- Store at controlled room temperature 15°-30°C (59°-86°F)
- Protect from light and excessive heat
- Keep bottle tightly closed when not in use
- Discard solution 4 weeks after opening (mark opening date on bottle)
- Do not freeze
- Keep out of reach of children and pets
- Do not use if solution changes color or becomes cloudy
