Azithromycin is a macrolide antibiotic indicated for the treatment of a wide range of bacterial infections, including respiratory tract infections, skin and soft tissue infections, and certain sexually transmitted diseases. Its extended half-life and tissue-penetrating properties allow for convenient once-daily dosing and shorter treatment courses compared to many alternatives. This medication works by inhibiting bacterial protein synthesis, effectively stopping the growth of susceptible bacteria. Proper diagnosis and prescription by a healthcare professional are essential for appropriate use.

Features

  • Active ingredient: Azithromycin
  • Drug class: Macrolide antibiotic
  • Available forms: Tablets, oral suspension, and intravenous solution
  • Standard strengths: 250 mg, 500 mg tablets; 100 mg/5 mL, 200 mg/5 mL suspension
  • Prescription-only medication
  • Manufactured under strict pharmaceutical quality standards

Benefits

  • Effective against a broad spectrum of Gram-positive and Gram-negative bacteria
  • Convenient once-daily dosing regimen improves patient compliance
  • Short treatment duration (typically 3-5 days) for most indications
  • Excellent tissue penetration with concentration at infection sites
  • Generally well-tolerated with favorable safety profile
  • Available in multiple formulations for different patient needs

Common use

Azithromycin is commonly prescribed for bacterial pharyngitis, acute bacterial exacerbations of chronic obstructive pulmonary disease, community-acquired pneumonia, uncomplicated skin and skin structure infections, acute otitis media, and urethritis/cervicitis caused by Chlamydia trachomatis. It is also used as alternative therapy for patients allergic to penicillin-class antibiotics. Off-label uses may include prevention and treatment of Mycobacterium avium complex in HIV patients and certain gastrointestinal infections.

Dosage and direction

The dosage varies based on infection type and patient factors. For most respiratory and skin infections: 500 mg on day 1, followed by 250 mg once daily on days 2 through 5. For sexually transmitted diseases: single 1 gram dose. Oral suspension should be shaken well before administration. Tablets may be taken with or without food, though taking with food may reduce gastrointestinal upset. Complete the full prescribed course even if symptoms improve earlier.

Precautions

Use with caution in patients with hepatic impairment. Monitor for signs of Clostridium difficile-associated diarrhea. May prolong QT interval; use caution in patients with risk factors for arrhythmias. Not recommended for patients with known QT prolongation or those taking other QT-prolonging drugs. Use during pregnancy only if clearly needed. Breastfeeding considerations: azithromycin excretes in human milk; caution advised.

Contraindications

Hypersensitivity to azithromycin, erythromycin, or any macrolide antibiotic. History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use. Concurrent use with ergot derivatives or drugs that significantly prolong QT interval. Severe hepatic impairment. Known hypersensitivity to any component of the formulation.

Possible side effect

Common: diarrhea/loose stools (5-14%), nausea (3-12%), abdominal pain (3-7%), vomiting (1-7%). Less common: headache, dizziness, rash, vaginitis. Rare but serious: QT prolongation, torsades de pointes, hepatotoxicity, Clostridium difficile-associated diarrhea, allergic reactions including anaphylaxis, hearing impairment. Most side effects are mild to moderate and self-limiting.

Drug interaction

Significant interactions with warfarin (monitor INR), digoxin (monitor levels), cyclosporine (monitor levels), and antacids containing aluminum or magnesium (separate administration by 2 hours). Contraindicated with ergot derivatives. Caution with other QT-prolonging drugs (antiarrhythmics, antipsychotics, fluoroquinolones). May increase concentrations of drugs metabolized by CYP3A4.

Missed dose

Take the missed dose as soon as remembered, unless it is almost time for the next scheduled dose. Do not double the dose to make up for a missed dose. Maintain the regular dosing schedule. If multiple doses are missed, contact healthcare provider for guidance.

Overdose

Symptoms may include severe nausea, vomiting, diarrhea, and temporary hearing loss. Management is supportive; gastric lavage may be considered if presented soon after ingestion. There is no specific antidote. Hemodialysis is not effective for removal. Contact poison control center or seek emergency medical attention immediately.

Storage

Store at room temperature (15-30°C or 59-86°F). Keep container tightly closed. Protect from light and moisture. Do not freeze oral suspension. Discard any unused portion of oral suspension after 10 days. Keep out of reach of children and pets. Do not use after expiration date.
